Health assessments

An integral part of General Practice is screening patients for chronic disease risk factors and intervening before they can develop. Health assessments are a valuable tool in this process and are available free of charge to 40-49-year olds, >75-year olds, Aboriginal and Torres Strait Islander people of any age, people with an intellectual disability and permanent residents of Aged care facilities.

Chronic conditions are the leading cause of illness, disability, and death in Australia according to the Australian Government department of health. These can include heart disease, cancer, and diabetes. Performing health assessments on specific target groups can assist in screening for common chronic diseases. During your health assessment your medical history will be updated, including past history, medications and family history. You will be weighed and measured, including blood pressure and heart rate. You will be asked questions relating to different body systems and tests may be arranged. You will also be provided with health education.
